In the July 2009, Vol. 5, No. 2 issue there is an article on Renewal Questions and Answers on page 11. See http://www.in.gov/pla/2489.htm.
You are required to disclosed pending State Nursing Board investigations, pending criminal charges, and criminal convictions that were expunged or resolved through criminal diversion.
The application also asks if you have been terminated, reprimanded, disciplined, or demoted in your scope of practice as a nurse or as another healthcare professional.
Nurses are encouraged in the article to err on the side of disclosure because Renewal Fraud is grounds for disciplinary action in Indiana.
